When you schedule an appointment, we will email you a brief intake form to ensure that we don’t have any conflicts, along with a link to pay the $100 fee. We also ask that you complete a Confidential Client Questionnaire, and submit any Orders or documents you want us to review in advance of the meeting. Please be advised that we will not be able to call you unless you have filled out the intake forms and pre-paid the consultation fee. The consultation will be limited to your concerns about how COVID-19 could affect you.
