Once the Order legally changing your name is signed by the Judge, you should first assure that you have at least one certified copy for your records. You may need this to show governmental agencies or financial institutions to change your name/address on accounts, transfer titles, etc. We have created this checklist to assist you in notifying entities of your new name.
You may want to consider creating a new email account prior to addressing the following changes if your current one is tied to your old name.
General:
- Update your driver’s license.
- Update your social security card.
- Update your passport.
- Update your Global Traveler, TSA Pre-Check accounts.
- Notify your employer.
- Notify post office.
- Update your vehicle registration.
- Update your voter registration.
- Notify utility companies; they sometimes require a new account to be opened. Also update online access portals.
- If you rent, notify your landlord.
- Notify your travel provider accounts (airline, frequent flier, hotel, car rental), online access portals.
- Update professional licenses.
- Notify telephone provider(s), online access portals.
- Change name and login information for online access portals to all retail accounts: stores, shopping sites, restaurants, home tech, media, shared ride sites, grocer/shopping sites.
Financial Accounts:
- Notify all financial institutions, including checking & savings, investment and brokerage accounts, including online access portals.
- Update retirement accounts, online access portals.
- Update credit card accounts, online access portals.
- If you close financial accounts and open new ones, update any automatic payments with any new account information.
- Update financial merchants (Venmo, PayPal), etc., online access portals.
- Update safe-deposit boxes.
- Monitor your credit report every 90 days until your new name is recorded.
- Create new Estate Plan documents (Will, Trust, Health Care Power of Attorney, Living Will & Durable Power of Attorney).
- Ensure all prior Powers of Attorney are revoked in writing. Prepare new ones if needed.
Medical:
- Update healthcare providers and update medical records (including HIPAA authorizations) with new name. Update online access portals.
- Update pharmacies, online access portals.
- Update HSA/FSA accounts, online access portals.
Insurance:
- Update health insurance provider.
- Update auto, home owner & flood insurance records.
- Update life insurance, disability or other insurance policies.
